Travel vaccinations

Please complete and submit the travel questionnaire at least eight weeks in advance of your holiday.  This will give the practice sufficient time to order the required vaccines and for the vaccinations to take full effect before you travel.

The Practice Nurse will review the completed form and determine which vaccines are required.  Not all vaccines are available on the NHS.  For vaccines that are available on the NHS, you will be contacted within 2 weeks of submitting the form to book an appointment with the practice nurse. 

For those vaccines not available on the NHS, you will be advised to contact a private travel clinic.
